COVID-19 Screening: Performers & Backstage Staff

All performers and backstage event staff are required to complete this screening. If you have completed your vaccination series, you will only have to complete this form the first time you come to work an event. If you have not completed a vaccination series, you will have to fill out this form every day you are scheduled to work prior to entering the theatre.

You will not be allowed to enter the theatre without filling out this form. You will not be allowed to enter the theatre if you fail this screening.
